Position: Service Coordination Supervisor

Qualifications:

• Bachelor’s Degree in Human Services field
• 3+years of Service Coordination or related experience
• Staff supervisory experience preferred

Skills Considered Necessary to the Position:

• Proficient Word-processing/Typing/Editing Skills
• Excellent customer service and communication skills
• Problem-solving and creative thinking skills
• Math Proficiency (basic math and algebra skills)
• Extensive knowledge of the Title XIX I/DD Waiver program

Pay Range: $40,000

General Description: The Service Coordination Supervisor is directly responsible for all aspects of service coordination service delivery within the assigned region. Must be able to work with the Director of Service Coordination to assure delivery of quality and economically viable services.

Responsibilities:

•    Provision of Service Coordination Services as stated in the Service Coordination job description.
•    Hire and supervise activities of all Service Coordinators.
•    Hold monthly Service Coordination Meetings (Staff Development).
•    Coordinate and supervise training of service coordinators supervised.
•    Assure all PAIS, Inc. policies and procedures are being followed.
•    Assure all Title XIX I/DD Waiver policies are being followed.
•    Maintain financial viability of service.
•    Assure (through tracking systems and review) quality and timeliness of all Service Coordination documentation.
•    Available for staff questions regarding budgeting, I/DD Waiver and PAIS policies.
•    Monitoring of utilization of Service Coordination services to assure provision of service as stated on IPPs.
•    Provide training and outreach per request of participating families, community organizations/agencies, staff and supervisor.
•    Complete plan of corrections for QA reviews.
•    Review referrals with Director of SC services.
•    Responsible for disciplinary action with Service Coordinators.
•    Complete staff performance evaluations for Service Coordinators per policy requirements.
•    Complete Quality Assurance activities and IPP Reviews per policy.
•    Perform staff billing audits per policy.
•    Assure all incidents of abuse/neglect are reported appropriately.
•    All other duties as assigned by Director of SC services.
•    Maintain current policy and procedure manuals in offices.
